- More blood and disturbing images than any other film in the franchise.
- Grisly crime scene images of a dead teenage girl are seen on a few separate occasions. Towards the beginning we see graphic images of open lacerations on her throat and on other occasions we see her blood soaked face and neck. We also see other images of another corpse with lots of blood and wounds these are too brief to describe.
- References to two people being brutally murdered in their beds sleeping. At one point of the film we see a ghostly image of a girl laying in her bed completely matted with blood. She also has open wounds and is covered from head to toe with blood as well.
- A man is seen missing both of his eyes and with blood pouring from his mouth. Both his eye sockets and mouth are leaking large amounts of blood.
- A woman is grabbed by a creature and killed. Another woman is lunged at and also killed in the same scene. Neither of their bodies are visible and their deaths are off camera.
- References to mass suicide, a killing of a family, an attempted kidnapping of a child amongst other things.
- A girl who is said to have died in a car accident is seen with gross facial wounds and in a ghostly form is pale and lifeless.
- Little violence but lots of disturbing images.

- The film is extremely scary throughout. There are claustrophobic scenes, moments where characters are getting tormented and killed by horrific looking creatures and there are disturbing references and images in relation to murders and suicides.
