When Halstead is drilling the hole in the mobster's head, the drill bit has blood on it all the way up to the chuck when he started drilling. Since he was operating the drill in a forward direction, the blood would not have been on the upper part of the bit as was shown, it would have been cast off as he drilled.
When the police officer tells Dr. Rhodes that Dr. Halstead called to tell him "It's all good", he should have said Officer Halstead, not Dr.
Not a goof: The police officer said "Dr. Rhodes? Detective Halstead asked me to forward a message. He says it's all good. Said you'd know what that means."

When James Young is recounting to Dr. Charles how he (James) met his future wife, he says he drove up to Pontiac on a whim in May 1987 to see a U2 concert.
U2 did play a concert in Pontiac, Michigan, in 1987 as part of their Joshua Tree tour, but the Pontiac concert took place on April 30. One could forgive someone for forgetting the exact date of something that occurred 31 years ago, but since James is telling the story because it holds so much significance for him, one would think that he would remember the actual month, especially since this is when he met his future wife, "the woman of my dreams."
