- Shaun's proposed treatment for a homeless patient puts him and Jared in Andrews' crosshairs. Meanwhile, Claire tries to overcome Melendez' reluctance to do a risky heart operation while Glassman must overcome his personal feelings about his oncologist, Dr. Marina Blaize, and face a difficult decision about his health.

Andrews starts his new position by giving performance reviews to doctors in St. Bonaventure hospital. Dr. Shaun Murphy has exceeded Andrews's expectations in terms of expertise and technical abilities. Still, he pays sincere attention to the lack of communication skills. Shaun Murphy and Dr. Jared Kalu run the hospital's community service to a homeless encampment. He asks the result of Shaun Murphy's review and Shaun suggests Kalu gets a review also, but Kalu thinks that the feedback will be meaningless since that day is Kalu's last day in the St. Bonaventure.
Waiting for Melendez, Dr. Claire and Dr. Morgan guessing at each other's review result, which is interestingly accurate. Pulmonary hypertension and heart failure, so the description of the patient by Morgan once Melendez comes and asking for the patient's condition. In fact, they are gonna conducted surgery for a patient with two hearts.
Back to the homeless camps, Shaun shares similar past life experiences with a homeless woman while checking her leg. He is suggesting the woman should move to a bus (wreckage) for more quality life. Kalu reminds Shaun that Glassman is no longer be with him all time and questions Shaun about why he does not accompany his long-time friend during his first-day treatment.
Glassman starts the day with debates with the oncologist Dr. Blaize as he wants to ensure everything works under his wish and quick. Still, Blaize insists that existing procedures should be followed, including waiting lists and a lot of tests preceding the surgery, regardless of the patient's professional background. Glassman finally relented as he has no choice.
After catching a homeless man sneak up into the hospital van, Shaun and Kalu are trying to calm him, named Harry, who already pick-up some pills. Responding to the doctors with disorganize thinking, paranoia, and delusion characteristics of expression as Shaun describes, this unfortunate man concluded by Shaun is having Schizophrenia and Chlamydia as well. Then the homeless man shows a stink boil in his upper gum.
Melendez team meet with their patient whose heart failing, Melanie. Her husband, Elliot, emphasizing how rare is the health problem that she has. Melanie interrupts Elliot and convinces him Melendez is the best cardiac surgeon and will give her ten more years to live. Melendez feels flattered, but he soon feels upset because it was Andrews who told her along with the press about the surgery and the chosen surgeon. In his room, Andrews explains that the move is for drawing funds for the operation in accordance with achieving his new vision for the hospital, surgical center of excellence.
Shaun gives first treatment and Kalu later finds a condition that exacerbating the man's deteriorating health and prepares to bring him to hospital, but Shaun is reluctant to put such a big stake. Kalu tells Shaun he took any responsibilities regarding his belief that Harry has Meningitis.
In the MRI room, Melendez team discussing the feeling loss of Jared. Dr. Park discovers that Melania's heart state worse than expected and seems like beyond help. In another room of the hospital, Kalu again persuaded Shaun the importance of being on Glassman's side at that time. Still, Shaun believes that his status as a friend to Glassy doesn't require a physical presence. Shaun worries about coming true as Andrews came to them and threatening to call Denver Memorial Hospital, Kalu's next workplace. Andrews thinks this treatment wastes hospital's resources.
At a consultation with Andrews, even though Claire, Morgan, and Park have presented a breakthrough, the Teflon graft for Melanie's treatment. Melendez rejects to undergo this surgery, citing some possible risks.
Jared realized his final day is ruined as Shaun reads Harry's test result is negative. He is back to the site to unload the van, and for the third time, he asks Shaun to visit Glassy. Still, Shaun chooses to finish his book chapter instead of accompanying Glassman. Meanwhile, in another place, Aoki tells Andrews he should learn from Glassman's manipulation method during his long presidency to deal with such a person like Melendez.
Shaun visits Glassman and encounters his mentor, He has been throwing papers to a bin while frustratingly waiting for his turn. Glassman said Shaun is gonna waste his time to be with him, but Shaun replies he can wait as well as finish his chapter book. Glassman continues throwing paper and Shaun look at the papers were scattered outside the bin, which means most of the thrown papers didn't go on target. Now, Shaun's brilliant imagination is started. He ripped a page in his book and positioning his hand directed to the bin. Imaginer lines are drawn between Shaun's sitting position and his hand to the bin along with mathematical formulas. Shaun recalls his memories when Harry was difficult to positioning his hand to grab a glass from Shaun during his first treatment in the van. Shaun then discovers his patients' actual disease is Kaleidoscopic Disintegration, caused by a brain tumor.
Shaun goes back to the hospital and stops Kalu from leaving the hospital. He needs Kalu to help him to pick Harry again from the encampment. The camp shows unfriendly welcomes to Shaun, but Kalu takes immediate action. Finally, they found Harry and also Harry's real name, Edward Austin Thomas. Shaun says that both he and Harry have a neurological condition, Harry's can be cured, but Shaun's not. Andrew tries a persuasive way against Melendez and it works. Melendez meets Melanie and ensures Elliot as the surgery will be conducted soon. Aoki and Andrew watch the surgery. Melendez team encounters a problem that the proposed steps of the surgery are impossible to be continued. Melendez then took the riskier step replacing the entire ascending aorta with Teflon Graft, not the small part one.
Andrews comes to the MRI room, where Harry's body scanned, with anger. Kalu elegantly replies that it was not him to bring back the patient for treatment but Shaun. Kalu explains that Shaun builds communications with Harry, successfully convinced him to back to the hospital, stick and struggle with his medical opinion. Shaun then confirms as revealed on the MRI screen, Harry has a brain tumor. Lead by Dr. Lim, Harry's surgery now finished, and now he is Edward. His family visits him and tells Shaun and Kalu why Edward was ended up on the street.
Melania's surgery goes well as both hearts are beating. Melendez stares up to Andrews as he proved he is the capable one to undergo this surgery. Claire convincingly answers Melendez's request to finish the operation. She said she started with the idea of this surgery, and she would be the proper one to end this surgery. She stares up to Andrews, signifying she is already addressing her lack of assertiveness, as mentioned during the review. Andrews then hold a press conference. Melendez's team success confirms his vision to establish a surgical center of excellence.
Jared says farewell to Shaun and tells Jared really appreciates every Shaun's efforts, which the president may not know. Before leaving the hospital, Jared says goodbye to Claire and leave her with tears.
Shaun meets Glassman, who is already waiting in the hospital lobby. Shaun said he will be with Glassman during his future long treatment. Arrived at his flat, Shaun sees Lea stands on the front door and fall silent. Lea asks Shaun at least to deliver a greet, and Shaun then says, "hello."
