- Reese and Finch become trapped in a hospital that becomes ground zero for a deadly viral outbreak. Also, Samaritan's newest recruit has second thoughts and Shaw continues to struggle with reality.
- Shaw plans and executes an escape from Samaritan's operatives. Mr. Reese is following an international business man named Ko who's getting sick. Things get worse when Ko dies as he may have been infected with a mysterious virus. As a result, Mr. Reese and Harold get trapped in a hospital trying to find the person who may be responsible for Ko's death as well as preventing an outbreak. From the subway, Root helps them identify the perpetrator. However, a third player comes around, Jeff, who's given a tough choice by an operative of Samaritan. Fusco reaches out to Elias to help him dig deeper into the dead bodies from the tunnel.—Andreea D
- Reese tails the next number from the Machine, that number belonging to an international businessman named James Ko, whose Hong Kong to London flight was diverted to New York for mechanical issues. Because Ko leaves the airport during the layover, Reese believes he is the perpetrator. But where he goes makes Reese then believe the Machine is mistaken that Ko isn't involved in anything nefarious. Reese and Finch will ultimately learn that Ko is neither the victim or perpetrator but rather an unwitting cog, only one of many, for the perpetrator to hit his/her intended target(s). Because of the intricacy of what is happening, the team knows that Samaritan is behind the operation. Reese and Finch, who comes on site to assist, not only have to save the intended targets, but prevent other deaths, prevent mass hysteria because of the situation and discover what Samaritan's goal is in killing the target(s). Meanwhile, Jeff Blackwell is determined to eke out a new life with his old girlfriend, however he is having second thoughts about the work he is doing for the mysterious woman who dictates his assignments. Fusco is still determined to find out the plot behind the deaths of the missing persons. Not wanting to involve the team especially his "partner" Reese, Fusco turns to someone else he figures must have some inside information on the issue. And Shaw believes she is in reality and not a simulation when she makes an escape attempt. If she is indeed in reality during this time, where she ends up may be just as difficult to escape from.—Huggo
- Shaw subdues her doctor and escapes through the walls of the hospital. Her destination is a cell in a prison, and she learns that she is Johannesburg. Jeremy Lambert finds her and says that it is another simulation. However, she kills him and flees. Meanwhile, The Machine delivers the number of the businessman James Ko, who is followed by Reese. Ko does not fell well and goes to a hospital. Dr. Mason diagnoses influenza and Nurse Carroll gives a shot of antiviral. After the injection, Ko walks and falls in the hallway. Dr. Mason pronounces him dead, and the blood test shows that he had avian influenza and the antivirus contained live influenza virus, becoming contagious and lethal. The CDC determines a lockdown of the hospital. Meanwhile, Fusco meets Elias and ask him to help in the investigation of the exploded tunnel, showing him that Bruce is dead. Elias gives the name of Frank Capello, who knows every truck in New York. Frank is pressed by Fusco and shows the photos of the drivers. Fusco identifies Jeff Blackwell, who was sent to the hospital by Mona to inject contaminated blood in Dr. Mason and Nurse Carroll.—Claudio Carvalho, Rio de Janeiro, Brazil
