Sex & Nudity

  • Brief references to sex are made.
  • The main character saves a woman from a film producer's mansion, it's inferred that she was drugged and raped, and that the main character and her start a relationship soon after.

Violence & Gore

  • A person can be seen having their eyes jabbed out in a bloody, disturbing way in one level
  • Enemies who are shot will sometimes been seen with entrails that have fallen out after they are killed. Bullet wounds are also graphically depicted on dead bodies.
  • Despite the violence, the graphics are very pixelated, which reduces the impact, but it's very graphic and brutal nonetheless.
  • Despite being pixelated, the game is incredibly violent. The player can preform graphic executions on downed enemies, including smashing their head with a bat, slamming their head against the ground, and ripping their jaw open with a claw hammer, all of which include large amounts of blood spraying.
  • Throughout the entirety of Hotline Miami, the player is tasked with killing all the enemies within a room, in very bloody and gory ways, through shooting, stabbing, and even bludgeoning.
  • One of the core mechanics has the player brutally finish off downed enemies through ways of rapidly smashing their heads against floors, slitting throats, beating their heads in with melee weapons, etc.
  • (ORIGINAL TEXT) the graphics are pixelated, the various ways of killing people has a very realistic feeling to it, and features lots of blood and gore, but the game is made from a top down perspective and all the graphics are pixelated. The game also allows the player to finish off knocked out enemies, by repeatedly banging their heads to the floor, slitting their throats, and in other ways. The game at one point also forces you to kill someone who is trying to crawl away and stating "It can't end now, not like this", and can only be killed by having the player bash his head in.

Profanity

  • Few uses of the words 'Fuck', 'Bitch', and 'Shit'.
  • Swedish profanities can be found spray painted onto walls, etc. in some levels.
  • (ORIGINAL TEXT) The game features relatively strong profanity such as 'fuck', 'bitch' and 'shit', but as dialogue is relatively rare, profanities are aswell. There is although some graffiti scattered over the levels which sometimes features profanity, some being in Swedish.

Alcohol, Drugs & Smoking

  • Some of the locations in the game refer to beer, and it's consumption, as well as showing people getting drunk, high, and throwing up in the backgrounds of these locations.
  • Brief references to Alcohol/Drugs.

Frightening & Intense Scenes

  • After you've murdered everybody in a level, there isn't any fanfare. Instead, the music abruptly ends, and you go outside to the players car. While you do this, there is uneasy ambience, and you walk through the corpses of the people you murdered. May disturb some.
  • The fact that this is rated PEGI 16 (most of the time equivalent to a Teen Rating) is completely absurd, despite being pixelated it's still VERY bloody and graphic and that shouldn't downgrade how violent this game is. Also keep in mind that games like Castle Crashers and Battlefront II get the same rating as this.
  • Hotline Miami is really gory, and may disturb some players.
  • Many scenes throughout Hotline Miami may disturb some players.

Violence & Gore

  • A boss fight ends with the player character almost killing but ultimately only badly injuring a person by swinging at his head with a golf club.
  • A guy in a rat mask kills the main character's girlfriend, and shoots him in the head, wounding him.
  • The player is given the choice to kill the man who wounded you and murdered your girlfriend, though it isn't considered canon that this actually happens.
  • The main character murders a driver hired to crash into a building and try to kill him by rapidly smashing his head into the floor, then lighting him on fire once he's dead.
  • A hostile woman and two panthers get killed later in the game by the main character, who's trying to kill a crazed mobster, who ends up shooting himself in defeat.
  • The game ends with the main character fatally shooting the mob boss in the head.
  • In the finale of the Post-Credits levels, the player has the option to murder the two janitors responsible for the phone calls.

Frightening & Intense Scenes

  • At various points you walk into a room with 3 different people wearing animal masks, they say disturbing things and there is intense and scary music in the background. Could frighten some people.
