Paul goes to visit Mike to talk about Paul's adopted and Mike's biological son Zack. Paul wants Mike to find Zack since Paul is interested in finding him regarding the shooting. Since we last saw him several years ago, the mentally troubled Zack had used his fabulous wealth left by his biological grandfather to buy a big house, score hot babes and do drugs. Now, the masion where Zack lived in is condemned and boarded up. Mike is surprised by the news that Paul thinks Zack tried to kill him. Mike says that the last he heard, Zack was in Idaho. Paul wants him to pass along the message that he's disappointed in him. However, Mike is revealed to know where Zack is and when he is alone, Mike makes a phone call to Zack, who is seen listless on a couch in a motel room, near lines of cocaine.
Our narrator from beyond the grave, Mary Alice, tells us that the past always finds a way to catch up to us as Keith's former girlfriend Amber watches him enter Bree's house. Amber gives Bree a call and Bree makes an excuse to go outside and meet her. Amber asks for a cashier's check instead for the money that Bree gave her the other day to leave town, and Bree agrees to meet Amber the next day. Amber gives Bree a picture of Charlie to give to Keith.
At the hospital, Susan learns that she'll be waiting at least three to four years for a kidney according to the donor list. She has her beeper and she's off. She runs into an old high school acquaintance Monroe Carter (Dave Foley). She tells him about her kidney and says she'll see him at the reunion, if she's still alive.
Meanwhile, Lynette visits with her drunkard mother Stella and her new husband Frank (Larry Hangman) at the assisted living facility. They ask Lynette to bring Tom and the kids up to take a family portrait. Frank tells Lynette that he has some bad history, with three families that aren't be speaking to him. Lynette begs off and Frank tries to bribe her with $500. Stella sends him off and she tells Lynette that Frank is about to do his will and they need to play ball. Lynette says to bring Frank back to her house.
Gaby is in therapy and not taking it very seriously. The therapist tries to get her to focus, specifically on her father who died when she was young. Gaby tries to change the subject by telling superficial stories about her modeling days. The therapist asks why she doesn't want to talk about her childhood. Gaby says the past is past and she's always been very shallow. The therapist tells her not to be afraid and to talk about her childhood. Gaby comes home and tells Carlos she loves it. He says he's glad it's working and that he was worried. Gaby won't talk about whether she's talking about the doll, but tells him that she'll be going to therapy three times a week so he'll have to do the carpool and cancel his golf game. He balks but she says if he wants her to get better this is how it has to be.
The next day, Bree meets Amber at the restaurant and her son Charlie is with her. He's being a little uncooperative and so Amber asks Bree to stay with him while she goes to cash the check. He runs off to play games in the arcade.
Susan's beeper has gone off and it turns out that Monroe wants to give her a kidney. Which is, of course, weird. Turns out he's a match. She asks him why he would do this. He says she was wonderful to him in high school, she's in trouble, and he wants to help. He then asks for a hug. He clearly thinks he's going to get more than that.
Bree watches as Charlie plays. And oddly, Keith walks in and claims she's busted, she likes pizza after all. Charlie asks for more quarters, and Keith is all "who is this?" He gives him some quarters and starts to play ball with him. It breaks her heart to watch them play and how good he is with a child he doesn't even realize is his own. Keith leaves for his job and tells Charlie to remember him when he's on the Lakers.
At the Scavo house, Tom sets up the camera as Lynette's mom and Frank show up. The kids wonder why they're doing this. "For grandma," says Tom. Frank immediately angers and annoys Tom with insults to the house, as well as complaining about the Asian driver they were stuck behind, and tries to rearrange her furniture to his choosing. Lynette tries to fight with him, he stubbornly refuses. They move the couch. They pose for the photo. Tom thinks they have to do it again because Frank's eyes are closed. But, actually, it turns out he's dead.
A little later, the kids get sent upstairs while Lynette tries to console her mother. Stella's mostly worried he hadn't changed his will and since it doesn't go into effect until tomorrow she wants Lynette to wait to call the police. Lynette is aghast, but her mom says it's millions of dollars and tries to bribe her with college tuition and braces for her kids. Lynette hangs up and agrees to wait.
Mike flashes back to a trip to visit Zack some months ago. He had come to ask for money due to his and Susan's precarious financial situation. Mike tells Zack that he had been busted by his drug abuse and moocher friends. He tells him about Paul Young being out of prison having been exhonorated for Felicia Tilman's murder, and Zack claims that he didn't know. Zack is acting weird and Mike is confused and concerned. Apparently Mike didn't realize Zack was high on drugs.
Back at the Scavo house, Lynette and Tom stare at Frank's dead body. Tom is creeped out, but Lynette says it's just a body. She and Tom muse about what money does to people and how it ruins relationships, like Frank and his previous families.
Meanwhile, Carlos gets a call about Gaby's appointment and it turns out that she's been going to the spa, not therapy. Carlos goes to the spa and pretends to be her massage therapist until he reveals himself. She says this is therapy: massage therapy, aromatherapy. Gaby says she doesn't need a psychatrist and the spa is really helping. Carlos tells Gaby that she is actually ignoring her problems. She says that makes it seem worse than it is. In a beat tone, Gaby promises Carlos that she'll make an appointment for a therapist.
The police/coroner arrive the next day at the Scavo house to take Frank's dead body away and the ruse works. Stella closes the door and says she's rich! Lynette's mom starts giving orders to everyone: a laptop, a motorcycle, etc. Lynette asks that they show a little respect since a man has died.
Mike goes to visit Zack, who comes at him with a knife. Zack's room is a mess and he seems crazed and paranoid. Mike imparts the message that Paul thinks Zack shot him. Zack confesses that he did and says it doesn't seem right that Paul gets to go on with his life after he ruined Zack's with the knowledge about his past, and his adoption which drove him over the edge. Zack tells Mark that he dumped the gun he used at Bree's house and realizes this was a mistake. "That's what happens when you're on drugs" says Mike. He tells Zach that he'll get him into rehab. Zack says it's too late for Mike to play daddy and it's his fault that Paul, "that monster" who raised him. Zack apparently blames his adopted father, Paul, for his entire upbringing and downfall as well as Mary Alice's suicide. Mike tries again to offer help and Zack kicks him out.
Meanwhile, Susan has Monroe over and gives him a small crystal heart as a gift. He says in high school he always hoped he'd get her heart. He gives her some old "memorabilia" from high school that didn't make the cut for the yearbook. Including tons of photos of her, an old English paper, and her, EW, retainer from the trash. He clearly had it bad for her and is borderline cuckoo.
Lynette's mom comes back o the house loaded down with shopping bags and a fur coat, money for the kids, and a thank you necklace for Lynette... worth $2,200. Stella is now drunk with money. She pushes Lynette into having the family over to the assisted living center for dinner. Lynette refuses, but Stella is basically blackmailing her. Lynette is aghast at what her monster of a mother has now become: an even bigger monster.
Bree comes home to Keith watching a game and having a beer. She tells him the truth about Amber and shows him the picture of Charlie.
Susan has a drink with Renee and Lee and tells them about selfless Monroe. They find the scrapbook and are freaked out by his obsession with her. She admits she's a little concerned that he's a stalker but she doesn't want to wait another six years. She wonders why she shouldn't do it. "Because she'll never get rid of him" says Renee.
Gaby asks Carlos if she has a fever, trying to get out of going to therapy. He won't let her wriggle out of it. So instead she tries to wriggle out the bathroom window at the therapist office, but Carlos is waiting for her. She says she'll take pills or see a hypnotist but she won't talk to a therapist, because Gaby does not want to talk about her childhood. Apparently, there's a dark secret. Carlos says she had the courage to talk about it once with him. And, she says, they agreed to never talk about it again. Carlos says he knows what keeping this secret in has cost her over the years and hears her cry in the bathroom at night sometimes. He says the therapist might help her let it all go. She asks him to come with her. He does.
In the therapy, Gaby says when she lost Grace it tapped into her need to protect children. The therapist asks where this need comes from. Gaby says that when she was a child, her stepfather molested her.
Paul is watering his flowers when Mike pulls up and tells him he'll take him to Zack. He explains that Zack is their son and he needs their help.
Susan comes out to find that Monroe is renting an apartment near Mike and Susan. He says he figures they should be close now. She says there's close and then there's Glenn Close and that this feels a little stalky. She points out that he's giving her a kidney. She says she's grateful but after the operation is over that's it, because he wants to be more than friends and she's married. She says if she still wants to give her one, no strings attached, she will be honored to take it and he'll have her eternal gratitude. Monroe says that's all he'll have though. She asks "what he says now?" He says he can't, breaks down in tears and says he thought if he did this she would finally love him. She says she knows and that she'll be fine. He leaves.
Epilogue. Mary Alice reminds us that the past has a way of catching up to us and we must confront it. We see Gaby talking in therapy. Keith looking at the picture of Charlie. Susan waiting in the hospital. Zack opening the door to his room to find Mike and Paul.
