Towards the end of the episode when Monk and Samuel are roughing up Kenneth Nichols and are holding photos of their wives and having Mr Nichols say the wives names, Monk's grip on Trudy's photo changes with each change of the camera angle.
In the scene where Monk and Samuel are having juice in Monk's apartment, Monk gets up to get a bag for Samuel to blow cigarette smoke into. The level of juice in Monk's glass increases during this.
The woman's body is in the wrong crosswalk. It is in the cross street to where she was crossing and in the opposite direction in which the car was driving. To get there, it would have had to magically pass through the car and be thrown behind it by some mysterious force.
