Sex & Nudity

  • Several sexual references and innuendos.
  • In one brief part of the game Max walks through a bar/brothel. Full frontal nudity and sex is shown in the background. You stay in the building for about five minutes, as a long cutscene plays and frequently pans between Max and pole dancers. There is a gunfight that stops this cutscene, and you are supposed to leave the building afterwards. However, you can still walk around and look at naked women hiding in closets and other rooms.

Violence & Gore

  • A man is set on fire and another is stabbed in the throat with a machete.
  • A man is killed with a scalpel. The action of it's off screen, but it can be heard briefly.
  • Gunshot wounds are among the most detailed in any game, especially on the highest graphical settings on the PC version of the game.
  • An innocent clerk is unexpected shot in the head by a sniper. Bloody and explicit depicted.
  • The favela raid by the police ( UFE ) is intense and brutal. Even though they are police officers, they show no mercy towards unarmed residents of the area.
  • In a cutscene, a woman is shot in the head by a man while he smirks about it. Surprising and disturbing scene.
  • In one scene, Max jumps on a man's back who is about to abseil down a river on a rope. On the way to the other side, soldiers come and fire countless bullets at the man while Max uses him as a protective shield. Very bloody depicted.
  • Very graphic, bloody violence. Whenever an enemy is shot there is a quite prominent blood spurt. Blood sticks to surfaces and pools under the bodies. The player must engage in shootouts with countless enemies which all become a bloody, graphic carnage.
  • Sometimes you follow the bullet as it impacts, similar to Sniper Elite V2 and Sniper Elite 3, but without X-Ray cam, and you can keep shooting. This's very gory and disturbing.
  • There are two scenes where an enemy is badly burnt and missing an arm and/or a leg. Both scenes are a bit disturbing.
  • You see a lot of dead bodies in a mission with blood scattered everywhere and you can see bodies in a hot tub.
  • A man's bone is exposed and is stomped on by another character, causing it to show outside of his leg.

Profanity

  • Most of the swears comes from the Brazillian character.
  • Uses of "caralho" can be heard.
  • Frequent use of fuck and variations of it.
  • Many other minor swears scattered throughout. Like shit, crap, damn and god damn it.

Alcohol, Drugs & Smoking

  • Painkillers also play a central part in gameplay, where Max heals himself by taking them.
  • There are several instances when drugs can be seen - in the favela and in the Police station.
  • Max and other characters can infrequently be seen smoking.
  • Frequent flash backs to Max drinking and abusing pain killers until he either passes out, or vomits.
  • Max Payne is addicted to painkillers. We see him taking pills and washing them down with hard alcohol, or simply drinking alcohol in more than half of the cutscenes.

Frightening & Intense Scenes

  • The imperial palace chapter is widely the darkest part of the game.
  • The most graphic game in the series.
  • Rated R for Strong Graphic Violence, Grisly Images, Language, Drug Content And For Some Strong Sexuality/Nudity.
  • The themes of kidnapping, torture (physical and mental), execution are present throughout the game.
  • Max is altogether a character with a wholly unhappy and relatively depressing past.

Violence & Gore

  • A man is executed by necklacing, and as he is burned alive you can hear him screaming. Very graphic and disturbing.
  • The final villain of the game is blown up, and is missing limbs and his skin is all burnt off as max watches him die.

Frightening & Intense Scenes

  • The plot revelation of human trafficking and organ harvesting could be considered as inappropriate, as it can be real where the story takes place.
  • Max is a very depressed character who abuses alcohol and painkillers. He makes a lot comments about killing himself since he lost his wife and baby. He thankfully seems to get over his depression by the end of the game as the main menu after beating the game shows him drinking soda instead of whiskeyl.
