Sex & Nudity

  • An female boss character is nude, with exposed breasts and genitalia.

Violence & Gore

  • At a certain point, the city becomes engulfed into a three-way struggle for control. There's chaos in the streets with pedestrians running for their lives while the military battles hordes of infected. As a result, corpses are littering the streets as millions are killed in the process due to the city becoming an active warzone.
  • Armored vehicles such as tanks can flatten cars, killing the occupants within. Civilians and mutants alike burst into fine paste when run over, leaving bloody stains and viscera on the ground.
  • The consuming process is extremely violent. For example, Alex can kill his victims stealthily by ripping their throat or beat them into a bloody pulp with his bare fists before assimilating the body.
  • This game is extraordinarily gory. Touted as an "absolute gorefest".
  • The aftermath is extremely bloody with people cut in half horizontal and vertical, both with their intestines clearly visible, and their bodies stay on the floor sprayed in blood for a duration.
  • Alex can shapeshift his body into weapons (e.g., blades, spikes, whip, mace etc) to slice, bludgeon, impale, eviscerate, and dismember enemies and sometimes non-combatants (e.g., pedestrians, police officers).
  • Blood splatter gushes out of injured victims, staining the ground and walls; body parts and dead bodies sometimes litter the environment as well.
  • During the frenetic combat, Alex can choose to 'consume' victims in order to replenish his health, take their memories or adapt their appearance and abilities.
  • Some abilities allow Alex to summon air strikes, pilot a helicopter, or drive a tank that can run over civilians.
  • The game's cutscenes contain real-world photos with digital manipulations of the following: open surgical wounds; a seagull pecking at a corpse; and various human fetuses in jars.
  • Alex can hijack the military vehicles and cause extreme chaos and bloodshed throughout the city.
  • The guns and rocket launchers, which are everywhere, are perhaps the least violent part of the game.
  • Alex Mercer has claws for hands and proceeds to use those to shred people into ragged, spurting bits, he can also cause spikes to rise from the ground, impaling foes, and send out tentacles from his body to skewer multiple enemies.
  • Every character onscreen is a potential target, including pedestrians (there's even a civilian body count report at the end of each mission).

Profanity

  • Extreme profanity, including not just frequent occurrences of words like "f-ck" and "sh-t," but several surprisingly creative variants.
  • Virtually every cutscene features multiple instances of these words.
  • Name calling (bitch and cunt).

Alcohol, Drugs & Smoking

  • Mention of experimental vaccines with adverse effects.

Frightening & Intense Scenes

  • Some memories can be quite distressing, not for the faint of heart.
  • Extremely bloody and gory.
  • There are some creatures that are pretty creepy looking, but not too frightening.
  • Some very disturbing live action images appear briefly during cutscenes. These images can be very upsetting for sensitive players, with such content as deformed children with no faces and close ups of surgical procedures.
  • Also as the game progresses the enviroment becomes very post apocalyptic zombie like.
