A man dreams that he is being chased by a murderess with an axe and blood on her face.

A man blows up a vehicle.

The head of a mental asylum sends security to capture a man alive, and he tells them to arm up in case he resists arrest.

A man shoots another man in the chest. It is revealed later in the same scene that this was not real, that he was imagining it, and that the gun was a squirt gun.

It is revealed that the protagonist killed his pregnant wife, which ended him up in the mental institution.

Profanity

There is quite a bit of salty language in the book, nothing too extreme. However, knowing Scorcese, his movies have tons of F-Bombs, who who knows.

Smoking is evident in the book, and some alcohol consumption.

While not a horror story, Shutter Island is quite intense and disturbing. The narrative is surreal and dream-like, and may be too intense for some. The trailer is falsely advertising it to be a horror movie, in case they threw in random jump bits to make it a horror movie. Some may find some of Teddy's nightmares to be intense
