A new coffee shop opens at Sacred Heart, providing employment for Cabbage, J.D.'s former intern, as well as an addiction for the rest of the crew. The janitor and his cronies petition Dr. Kelso for dental coverage and resort to taking over the coffee shop (which provides dental) to make a point when he refuses. Meanwhile, J.D. and Kim continue to move forward in their relationship while J.D. and Turk deal with an ethical dilemma when they begin treating patients for cash outside of the hospital. Elliot and Dr. Cox butt heads over a doctor patient who insists on treating himself.
Turk eventually reveals to J.D. that his stinginess has been due to the fact that he is afraid Carla won't go back to work now that she's had the baby and he will have to be the sole provider for the family. J.D. and Turk assure each other that everything will be fine. When the Janitor and his crew refuse to serve Dr. Kelso at the coffee shop, he caves and gives them their dental coverage. Elliot's doctor patient offers her a spot at his private practice because he was impressed that she stood up to him, and she accepts. Kim gets offered a new job in Tacoma, Washington.
