- Continuity: After Leon has his tongue ripped off, you can see the tongue intact in his mouth.
- Crew or equipment visible: When Maya is searching through Mahogany's bathroom, at one point the white t-shirt of a crew member is visible in the mirror.
- Anachronisms: When Leon is showing Maya the newspaper article dated December 19, 1895, a closeup of the newspaper shows a column of copy containing the words, "bikini-clad babes and tanned hunks". Putting aside the unlikelihood of that style of news-writing in 1895, the term "bikini", as regards clothing, was not coined until the mid-1940's.

- Factual errors: SPOILER: When a woman gets her head ripped off, she is still able to groan. This is impossible because she'll need her lungs to make that sound.
- Factual errors: SPOILER: There is no possible way in modern day New York that a train could operate on the current MTA subway rails without the notice of the central MTA transit operations center. All trains and tracks are monitored in real time electronically. If a train were to even miss a stop or a conductor switch tracks (which a driver could not technically do from within the cab), increase normal speed, etc, it would automatically be flagged at the central transit center, and the transit police would be notified. The system can also detect rogue or unauthorized trains on any of its tracks.
