• Continuity: When Mitch and Max are talking and Mitch takes the scalpel, he puts it in his left pocket. Later, when he stabs John with it, he takes it out of his right pocket.

• Factual errors: When John attempts to kill himself, he pulls the trigger and the hammer falls, but the gun does not go off because he says the safety is on. When a Beretta's safety is on, the hammer cannot be in the cocked position and the trigger mechanism is disabled, thus the hammer could not have fallen and pulling the trigger would have had no effect anyway.

• Errors in geography: This movie is supposed to take place in the U.S., but watch the scene where John Q. is being driven away at the end. If you look at the intersection ahead, you see a Toronto Transit Commission Wheel Trans bus go by from left to right. This is a bus for people in wheelchairs used in Toronto, Canada.
  
  
• Revealing mistakes: When the donor heart is being prepped for surgery, the dead woman's mouth twitches into a smile briefly.
  
  
• Revealing mistakes: When John Q puts the gun against his head and subsequently in his mouth to convince the doctor to operate, he yanks the gun down and back up to cock it, and the sound can be heard. However if the gun is unloaded as it is soon revealed to be, the slide would lock in the back position and stay there, but when John pulls the gun back up, the slide is still forward.
