  • Sal Piro, president of the National Rocky Horror fan club, has a brief cameo in this film. He stands below the staircase, talking on the telephone in the opening sequence.

  • The original script took place in natural locations around the town of Denton (Brad and Janet's house, Cosmo and Nation's "Hospital," etc.), and was set to be filmed in the real-life town of Denton, Texas, but the 1979-1980 Screen Actors Guild strike meant that no American actors would be available for the shoot, as no American actor would be allowed to act, at any location. This left the UK as the only filming option. The idea to have the entire film take place inside the Denton television studio was conceived by director Jim Sharman because no locations could be found in England to pass for an American suburb.

  • The "Lullaby" sequence was filmed in only one take.

  • The Transylvanian Thunderbolt symbol (from The Rocky Horror Picture Show (1975)) is visible next to a "High Voltage: Warning" sign on the wall of Janet's dressing room during "Little Black Dress."

  • The red throne on the "Faith Factory" stage is Frank N' Furter's throne from The Rocky Horror Picture Show (1975) - painted red.

  • There is a Time magazine in the Research Lab that has the Rocky Horror lips on the cover and reads: "Cult Movies."

  • Barry Bostwick had other filming offers and could not appear in the movie to redo the role of Brad Majors.

  • Tim Curry was offered the role of Brad Majors and Farley Flavors but he turned it down because he didn't think he could handle the American accent.

  • When the movie was released, it only played at midnight.

  • Richard O'Brien, Patricia Quinn, Nell Campbell, Charles Gray, Imogen Claire, and Jeremy Newson are the only cast to return, but Jeremy Newson is the only one of the six to reprise the same role ("Ralph Hapschatt").

  • Originally going to be called "The Brad and Janet Show"

  • One working idea for the movie was that Frank-N-Futer was going to come back from the dead and travel to Denton to find out Janet was pregnant with his child. Brad and Dr. Scott were supposed to have become lovers, and discovered a way to revive Frank using virgin blood. When he returns to Denton, Frank attempts to convert the entire town into a new cult of Transylvanians, and Riff-Raff and Magenta were to return to kidnap the child and destroy Frank once more. The idea was dropped when Tim Curry declined to participate.

  • According to the costume designer Sue Blane, in "Shock Treatment: Shockumentary", she bought the clothes of the audience from thrift stores to give them a cartoonish look and feel.

  • Susan Sarandon declined to return as Janet over salary disputes.

  • The role of Bert Schnick was originally written to be Dr. Everett Scott. The part was re-written when Jonathan Adams declined to reprise the role.
