  • Continuity: When Winslow is discovered by policemen in Swan's garden, the flashing red light seems to be on the left both of policemen and Winslow.

  • Continuity: At the airport, one of the "Undeads" has a bandage on his hand and a splint on one finger. During the rehearsals he is wearing a full sling and (apparently) a cast on the arm. During the actual performance in the next scene he has no splints, casts or sling.

  • Crew or equipment visible: There's a shadow of the camera and the cameraman visible on the blue car at the end of the "bomb in the trunk" sequence.

  • Continuity: There are only two scenes in the film where you can actually see Winslow's scars under the mask, specifically on his chin. One is when he first attacks Swan in the hallway. The other is with Phoenix on the roof. At all other times in the film, there is no scarring visible anywhere on his face.

  • Continuity: Swan is sitting watching his tape, after the car blows up on stage and sees the Phantom for the first time, in a black suit with a red shirt. When he comes out of the room and sees Winslow he is wearing a tan suit with a white shirt.

  • Errors made by characters (possibly deliberate errors by the filmmakers): In the bathtub, Swan mentions that Saturday, November 19, 1953, was the day he wanted to commit suicide. 19 November 1953 was on a Thursday.

  • Continuity: When the Phantom attacks Beef with the plunger, as Beef is sliding down the wall of the shower, you see the water spray of the shower twice, meaning two different takes were used for one scene.
