- Crew or equipment visible: Shadow of camera clearly visible after Carter slaps Margaret on the bridge.
- Audio/visual unsynchronized: Tightening of the screws on the coffin.
- Continuity: When Jack Carter arrives at Cliff Brumby's house you can see people at the party through the window. As the car door opens and he gets out, the people visible through the window have changed.
- Crew or equipment visible: In the ferry terminal shootout sequence, during the dialogue between Carter and the henchmen, a cameraman is reflected in the window of the ferry.
- Continuity: On more than one occasion but especially as she leaves the bingo hall to walk home, Margaret's shoes keep changing. They vary between beige mules and black patent sling-backs.
- Continuity: During the nightclub scene where Jack is tracking down Thorpie; Thorpe pushes past two men coming up from the toilets/cloakrooms. When Jack arrives in the toilets moments later one of the men Thorpe passed on the stairs going to the toilet can be seen combing his hair.
- Continuity: In the scene when Jack Carter goes into the cafe and asks Geraldine "Where's Albert", you can see the rear end of Kinnear's big blue Cadillac car parked in the street outside.
- Continuity: When the police raid Kinnear's mansion one of the police vehicles that drives in is strangely the same blue Land Rover that Eric, Peter and Con used in several scenes earlier in the movie.

- Continuity: SPOILER: At the end of the film, as Carter is shot in the head, his shotgun falls from his hand and lands away from his body, however in the next shot we see his body (lying at the edge of the water) the shotgun is clasped in his hand.
- Revealing mistakes: SPOILER: After Carter stabs Albert there is no blood on his knife.
