- After the collapse of his previous group-therapy practice, Richard is no longer able to conduct the traditional 50-minute sessions most therapists have with their patients. Instead, he's developed a new form of therapy: weekly quick-fire sessions with his patients that take place online, through a webcam. The few patients on his list present a hilarious, outrageous catalogue of neuroses, phobias, issues, anxieties, and psychopathies, but even these short sessions, where he's confronted by everything from the sublime to the ridiculous, are not his greatest challenge. His patience and temperament are stretched to their limit by the daily interruptions of a demanding extended family, errant teenagers, and his own trouble-filled past.

However, when Richard's testy father Jeremy demands he takes in his alcoholic mother Richard needs help from his own therapist Leonard.
- New client Nathan presents Richard with a problem close to home whilst Jeremy drops a bombshell. After Richard takes a call from unhappy lottery winner Terry his mother Maggie arrives, to the consternation of Karen and their children, while Jose is having trouble finding his way around the house. To escape the chaos Richard flees to have a less than eagerly anticipated vasectomy.
- Having run over a stray dog Richard is further stressed by his student daughter Izzie's affair with Nathan and Maggie's disruptive presence. To make things worse the dead dog has repercussions on a vulnerable client and Karen, unaware that he has backed out of having his vasectomy, has a work crisis. Leonard is no help either but, for all that, Celia seems pleased with his progress.
- Soldier Jon returns for Richard's son Ricky's birthday party and to more problems for Richard. Not only has he failed to split Izzie and Nathan but fear of reprisals from Neil leads him to extreme actions - re-mortgaging the house and trying to buy a gun on the dark web. To cap it all Karen is taken ill in Zurich and Leonard wants no more to do with him.
- Hung over after Ricky's party Richard can do without a snap supervision session from Celia, a failure to re-mortgage the house and a new patient with foreign accent syndrome. One client has good news, another claims Richard is his long-lost father whilst Nathan's arrival at and Maggie's disappearance from the house add to the chaos. Fortunately Karen and - inadvertently - Neil provide positive cheer.
